Under the direction of leadership, this position specializes in the clinical management of breastfeeding, lactation, and patient care coordination. The role advocates for breastfeeding as a global health imperative, creating supportive environments for breastfeeding families, while also focusing on care management to ensure high-quality, cost-effective healthcare across the continuum of care. What does a typical day on the job look like? Provides specialized education and support to breastfeeding and pumping parents, developing individualized care plans that reflect evidence-based breastfeeding practices. Assesses and intervenes as needed based on the health status of both mother and child, ensuring ongoing evaluation and support. Organizes patient consultations and breastfeeding group meetings, promoting timely and high-quality care. Serves as a resource for families, offering guidance on breastfeeding techniques, milk supply management, and equipment use (e.g., breast pumps). Educates parents and caregivers on the benefits of breastfeeding and addresses challenges, including latching difficulties, pain management, and supplementation needs. Provides transitional care management, ensuring follow-up within 7-14 days post-discharge, completing medication reconciliation, and addressing post-discharge needs (e.g., home health, DME). Longitudinal care Management - Develops a caseload of high-risk patients identified through frequent utilization or elevated risk scores. Personalizes care plans with patient-centered goals and provides education to improve compliance and health outcomes. Collaborates with interdisciplinary teams and external providers to meet patient needs efficiently and reduce barriers to care. Utilizes EMR systems and payer-provided tools to monitor population health and document patient interactions accurately. Ensures timely communication with patients, caregivers, and healthcare teams through phone, electronic, or in-person interactions. Collaborates with other providers and facilities to meet patient needs efficiently. Coordinates pediatric care services and ensures accurate documentation in the EMR. Provides administrative and clinical support by planning, organizing, and coordinating activities to meet patient needs. The role requires close collaboration with interdisciplinary teams, including physicians, nurses, and external providers, to improve patient outcomes and ensure efficient healthcare delivery. What are the requirements for the job? Bachelor's degree in nursing Current RN (Registered Nurse) license to practice in the state of Missouri. Minimum 1 yr experience in Pediatrics and NICU preferred Basic Life Support certification through American Heart Association. IBCLC (International Board-Certified Lactation Consultant) Required Critical thinking skills to identify high risk situations and prioritize care. Basic office skills: patient centered mindset, excellent time management and organization skills; ability to multitask and work in fast paced environment.
