Collaborates with the health care team to coordinate client and family interventions across the continuum of care, removing barriers and promoting efficient and effective use of resources. Assists clients and families in establishing client specific goals, both long term and short term. Facilitates appropriate follow up care and/or refers clients (within the designated client population) to the appropriate next level or site of care. Serves in an advocacy role on behalf of clients, families, and caregivers to ensure safe, equitable healthcare provision. Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the care coordination model. Assists in the nursing component of developing and meeting key Pillar goals including finance, satisfaction, clinical quality of care, innovation, and growth across the continuum of care.
30% Risk Reduction program coordination and oversight, including targeted programming for the 5 major lifestyle characteristics, Nutrition, Physical Activity, Weight Management, Tobacco Use, and Stress.
Development, implementation (with manager), and evaluation of programs

20% Provide health and lifestyle coaching
20% Hire, train, and supervise 2 health educators and 1 activities coordinator
10% Self-care development, including workshops (Babies & You program coordinator), Gaps in Care, materials development, and database updates (eg My Health Advisor)
5% Liaison to PCPs/Disease management program contacts
10% Collaborate on Go for the Gold program component development/marketing as needed
5% Participates as a Health Plus /Health & Wellness team member in staffing of Health Plus events/programs/facility, attending annual conferences/trainings, and other duties as assigned.
Direct supervisory responsibilities for 2 full-time health educators
Health Plus provides health promotion and prevention services that promote healthy lifestyles 24 hours a day, at home and at work. The Health Plus offices are located inside the new Vanderbilt Recreation and Wellness Center; services are provided at multiple locations throughout campus and well as at One Hundred Oaks and Williamson County.
